- Only two vendors publish real prices. Open Dental is $199 per location per month for the first year, then $149. CareStack starts at $829 per month (Essentials) and $1,299 (Intelligence, with AI). Everyone else quotes.
- Server or cloud is the first decision. Dentrix, Eaglesoft and self-hosted Open Dental run on a server in your office. Dentrix Ascend, Curve, Denticon, CareStack, Oryx and tab32 are cloud.
- AI is now a PMS feature. Dentrix Ascend, CareStack and Oryx build in AI notes, voice perio or x-ray checks. Open Dental sells native AI x-ray analysis for $149 per month; Dentrix and Eaglesoft rely on add-ons and partners.
- Best value: Open Dental. Best cloud for independents: Curve. Best for DSOs: Denticon or Dentrix Ascend. Most built-in AI: Oryx or CareStack Intelligence.
Practice management software (PMS) runs the whole practice: the schedule, the chart, treatment plans, insurance claims and the ledger. It is also where AI now shows up first. Voice perio charting, AI clinical notes, instant eligibility and x-ray analysis are increasingly sold as PMS features rather than separate tools. This guide compares the nine systems US practices most often shortlist. We checked every price and hosting claim against the vendor’s own site in October 2026.

1. Open Dental: best value
Open Dental is the only major system that publishes its whole price list. In the US it costs $199 per location per month for the first 12 months, then $149 per month month to month. That covers the software, every update, and phone, chat and email support for up to 3 providers; each extra provider is $20 per month. Add-ons are priced individually: the eServices bundle is $165 per month, eClipboard is $45, and ODMobile is $35. Data conversion is extra.
It is self-hosted by default, so you need a server and someone to maintain it. Open Dental Cloud removes that, starting at $250 per month for 7 concurrent sessions plus a one-time $400 migration fee. It is in limited release, and some third-party integrations don’t work with it. It is also the only PMS with a published price for FDA-cleared AI x-ray analysis: Better Diagnostics runs natively in the Imaging Module for $149 per location per month. Open Dental’s open database and API make it the easiest system to connect to other AI tools. Pearl, Overjet, VideaHealth and Better Diagnostics all integrate with it. See our Open Dental pricing breakdown.
2. Dentrix: the default for established practices
Dentrix from Henry Schein One is the most widely installed PMS in US dentistry. That matters more than it sounds: most hires already know it, and nearly every dental vendor integrates with it. It runs on a local Windows server. Henry Schein One sells add-ons on top: Eligibility Pro for insurance verification, eClaims, and Detect AI for radiograph analysis (powered by VideaHealth). Pricing isn’t published. Expect a license or subscription plus the Connected Care Essentials support plan.
Choose Dentrix if you run a single location, have a server you trust, and want the least retraining. Compare it with Open Dental and Eaglesoft.
3. Dentrix Ascend: best cloud system from a major vendor
Dentrix Ascend is Henry Schein One’s cloud system. It is a separate product from Dentrix, not a hosted copy, so moving from Dentrix is still a conversion. Its AI is built in and sold by package:
- Image Verify rates x-ray quality at capture so you can retake before a claim is rejected.
- AI voice dictation turns spoken notes into structured, claims-ready fields.
- Instant eligibility writes benefits straight into coverage tables.
- VideaHealth’s FDA-cleared detection is included as Detect AI.
Online booking and digital forms are native. It is a strong fit for groups that want one cloud system with shared reporting. See Dentrix vs Dentrix Ascend and our explainer on Dentrix Ascend’s AI features.
4. Curve Dental: best cloud system for independent practices
Curve Dental is browser-based and has been cloud-native from the start. Scheduling, charting, imaging, insurance, billing and patient engagement all share one patient record. Curve reports more than 100,000 active users in the US and Canada. It includes AI x-ray analysis and an in-product assistant, CurveAI. Conversion, training and support are handled by Curve’s own staff. Pricing is quoted by practice size, and some services are US-only. Curve is the natural choice for a startup that wants to skip buying a server, or for an established office leaving Dentrix or Eaglesoft.
5. Eaglesoft: best for Patterson offices
Eaglesoft is Patterson Dental’s server-based PMS, which Patterson says nearly 30,000 users rely on. It needs a physical server. Remote access is through the mostly view-only Eaglesoft Mobile app or a remote-desktop tool. You don’t buy the license on its own: it comes with a Service Club plan on a 24-month enrollment. The plan includes upgrades, unlimited support, Schick sensor support and discounts, including on Pearl’s AI. If your practice already buys equipment and supplies through Patterson, Eaglesoft fits neatly. If you want the cloud, it doesn’t.
6. Denticon: best for DSOs
Denticon from Planet DDS is a cloud system built for organizations with many locations. Its strengths are central visibility and control: native analytics, a dashboard bundle (morning huddle, production and collections analysis), and central insurance verification and claims. Clinically, it has integrated imaging, hands-free voice perio charting and an ortho module. Planet DDS’s implementation team is used to large, multi-system data conversions. For one location it is usually more system than you need.
7. CareStack: best published pricing for a cloud PMS with AI
CareStack is a cloud all-in-one PMS with two published plans. Essentials starts at $829 per month. Intelligence starts at $1,299 per month and adds AI clinical notes, AI voice perio charting, AI x-rays and AI clinical diagnosis. The final price depends on locations, chairs and providers. VoiceStack, its AI phone system, is sold separately. CareStack makes sense for a growing group that wants AI inside the PMS and can use its open API. A smaller practice should price Intelligence against a cheaper PMS plus a separate AI tool.
8. Oryx: most AI built into the PMS
Oryx comes in three tiers: Pro, Automate and AI. The AI tier has more native AI documentation than anything else on this list:
- AI clinical scribe
- AI voice perio charting
- AI exam charting
- Auto-generated exam notes
For imaging AI it lets you choose Pearl Second Opinion or Overjet. Oryx advertises “$0 to start” with no add-ons or hidden fees, and includes onboarding plus data and image conversion. Tier prices are quoted.
9. tab32: best for trying cloud before you commit
tab32 is a cloud PMS that runs eligibility checks automatically before each visit. It sends claims with e-attachments and includes voice-powered perio charting. It is the only system here with a published free trial: 14 days, cancel with one click. Use the trial on a real day’s schedule and claims before you sign.
How to choose
- Lowest cost and you have IT help: Open Dental.
- Single location, staff already trained, server in place: Dentrix, or Eaglesoft if you are a Patterson office.
- Leaving your server for the cloud: Curve for an independent practice, Dentrix Ascend if you want to stay with Henry Schein One.
- Three or more locations, or a DSO: Denticon or Dentrix Ascend, with CareStack as the alternative with published pricing.
- You want AI documentation without buying a separate scribe: Oryx or CareStack Intelligence.
TIP
Check AI integrations before you sign
If you plan to add AI x-ray analysis, confirm the PMS integration first. Pearl lists Dentrix, Eaglesoft, Open Dental, Curve and CareStack. VideaHealth is built into Dentrix Ascend. Overjet integrates with Dentrix, Eaglesoft and Open Dental. Our best AI tools for dental practices guide and FDA clearance tracker cover the imaging side.
What to ask every vendor
- What is the all-in monthly price for our locations and providers, including support, updates, eServices and texting?
- How long is the contract, and what does it cost to leave?
- What does data conversion cost, and does it include images and perio history?
- Which AI features are included, and which are add-ons?
- Does it integrate with our imaging software and our AI x-ray vendor, on our version?
Frequently asked questions
What is the most used dental practice management software?
Dentrix (Henry Schein One) has the largest installed base in the US, followed by Eaglesoft (Patterson) and Open Dental. Among cloud systems, Dentrix Ascend, Curve Dental and Denticon are the most established.
How much does dental practice management software cost?
Published prices range from $149 to $199 per location per month for Open Dental (up to 3 providers) to $829 to $1,299+ per month for CareStack. Dentrix, Eaglesoft, Curve, Denticon and Dentrix Ascend quote by practice size, and add-ons such as texting, eligibility and AI are usually extra.
What is the best cloud-based dental practice management software?
For independent practices, Curve Dental is the most mature cloud-native system. For multi-location groups and DSOs, Denticon and Dentrix Ascend are the main choices. CareStack and Oryx are strong if you want AI documentation built in.
Is Open Dental really cheaper than Dentrix?
Yes, on published pricing. Open Dental is $199 per location per month for the first 12 months and $149 after that, including updates and support for up to 3 providers. Dentrix pricing is quoted, but practices typically pay more once support and add-ons are included. Open Dental is self-hosted by default, so budget for a server and IT, or for Open Dental Cloud from $250 per month.
Which dental PMS works with AI x-ray software?
Pearl, Overjet and VideaHealth integrate with the major systems. Pearl lists Dentrix, Eaglesoft, Open Dental, Curve and CareStack. VideaHealth ships inside Dentrix Ascend as Detect AI. Oryx offers either Pearl or Overjet. Always confirm the integration with your exact PMS version.
